We have an exciting opportunity for an experienced RN to join our growing Population Health team! The Care Transitions Nurse plays an integral role in coordination of care for patients as they transition from an inpatient setting back to home safely.

The Maine Medical Partners Population Health Team care transition intervention is designed to serve the dual purpose of preventing adverse health events during various types of transitions; including avoidable readmissions upon transition from hospital to home and closing the gap between hospital discharge and the return to the outpatient environment.  During the patient outreach, the Care Transition team member reviews the current status of the patient, provides education regarding condition self-management and the identification of red flags to report.

  • Position Summary
    • This position is responsible for providing ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual (adult internal medicine and family practice) patient needs following discharge from the hospital.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: Graduation from an accredited RN program with preferably four years of experience in a hospital or medical office setting.
    • License/Certifications: Current, unencumbered RN license in the State of Maine.
    • Experience: Experience working with computers and an electronic medical record.
    • Good communication and interpersonal skills necessary for interacting with patients, physicians, supervisors and other staff.
    • Ability to organize and accomplish multiple tasks and to have the flexibility to meet changing demands on a daily basis.
    • Ability to maintain a professional manner even under stressful situations.
    • Comprehensive knowledge of medical procedures, tests, and clinical/clerical skills necessary to perform all functions and assist physicians.  Knowledge of pharmacology is desirable.
    • Analytical ability required in order to gather and interpret information necessary to resolve problems in an accurate and timely manner.  Ability to concentrate and pay close attention to detail.
    • Ability to maintain confidentiality.
