Summary

  • Position Summary
    • The ED Nurse Navigator (RN) assists patients manage the complexities of post discharge care by providing personalized, professional and compassionate care and follow up to Emergency Room and Inpatients care. The ED Navigator will work closely with care management and patient care services to facilitate high quality and timely follow up.  Care Manager experience is a plus.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: A minimum of Associates degree in Nursing; Bachelor Degree in Nursing preferred.
    • License/Certifications: Current license by the Maine State Board of Nursing, BLS and CPI required
    • Experience: Three to five years of experience in an acute health care setting preferred. Prior navigator and/or case management experience desired. Experience working with Emergency Department patients/families preferred.
    • Ability to function independently and solve problems in a complex environment, working with various members of the health care team is required.
