A Behavioral Health Care Manager (BHCM) is a generalist master's Level behavioral health provider whose primary role is focused on building and delivering the Collaborative Care Model (CoCM) within a medical setting. This a full time position, with the option of part-time hours for a candidate looking for a lighter schedule located at our Westbrook MaineHealth Primary Care Internal and Family Medicine, a well-established practice that offers complete care to adults and children.
The CoCM is an integrated behavioral health model of care aimed at achieving improved health outcomes for a population of patients while simultaneously targeting access. The behavioral health care manager (BHCM) is a core member of the Collaborative Care Model working in collaboration with the medical team, consulting psychiatrist, and behavioral health clinicians in the practice. Essential functions of the role include providing behavioral health treatment, supporting and coordinating the mental and physical health care of patients on an assigned caseload with the care team, consulting to and with the care team, linking and referring to additional services as needed, and engaging in additional activities as outlined by the model.
Supervision, connection to a larger behavioral health integration team, quarterly trainings, mentor and peer connections are additional benefits of this position.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: N/A
- License/Certifications: Licensure by the Maine State Board of Social Worker Licensure as a Licensed Clinical Social Worker (preferred), or licensure as a Licensed Clinical Professional Counselor (by exception). Must have a valid Driver’s license and reliable transportation.
- Experience: 2+ years of work experience in a health or mental health setting preferred, in order to have proficiency in psychosocial assessment and treatment. Must have considerable knowledge of community resources and agencies and how to use them in resolving complex problems related to health, illness and disability
- Knowledge of social work theories, therapies and techniques, as used in individual, family and group treatment, as well as principles of social welfare. Ability to teach basic social work principles and applications to the medical setting
