We have an exciting opportunity for an experienced Registered Nurse (RN) Care Manager to join our growing Population Health team! The RN Care Manager will work with patients whose chronic conditions are poorly managed and that impact their quality of life, symptom management and utilization of services. The RN Care Manager connects with patients by telehealth, phone, home or in the practice visits and supports them through the management of their complex medical needs, such as COPD, HF, CKD and HTN. As a trusted member of the health care team, the RN Care Manager works with the patient to identify goals that improve quality of life, patient experience, clinical outcomes, and reduce avoidable ED/Hospital utilizations.
The RN Care Manager will review the patient’s status, provide ongoing education regarding condition self-management, identify Social Driver of Health (SDOH) related barriers, place community resource referrals and bridge the coordination of services. This position is a full time and remote with occasional on site, practice visits and home visits.

We are looking for a Registered Nurse who will provide care Monday – Friday, day shift schedule, 40 hours a week. Our ideal Registered Nurse will hold and active license and BSN degree (although, not required).
Summary:
The Registered Nurse - Care Manager role is responsible for managing high-risk patients to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes, as well as ensuring safe and effective transfers in the movement of patients across the health care continuum, serving as the bridge between the professional staff in a care setting (e.g. hospital) and the patient and/or family.
