The ideal manager for this position will be a Registered Nurse with 5+ years of leadership experience who holds a BSN or ASN with added BS/BA in a health care. This position is primarily remote, but travel may be needed at MaineHealth locations though out the organization. This position is Monday - Friday, no weekends or on call required.
Summary:
The Population Health Manager is responsible for the operational oversight of the Population Health program at Maine Medical Partners.
Required Minimum Knowledge, Skills, and Abilities (KSAs):
- Education: Bachelor’s degree in nursing or an Associate’s degree in nursing plus a BS/BA in a health care related field; or a degree in Social Work.
- License/Certifications: Current, unencumbered Maine RN license OR Licensed by the Maine State Board of Social Work required.
- Experience: Prior population health experience preferred. Minimum of 3 years' ambulatory care experience required.
- Mastery of theories, principles and methodologies underlying population health and the patient centered medical home preferred.
- Strong background in health promotion, chronic condition management, care coordination, motivational interviewing, care/case management and care coordination preferred.
