Join our dynamic team as a Care Coordinator - a key role in the successful implementation of the Certified Community Behavioral Health Clinic (CCBHC). A Certified Community Behavioral Health Clinic (CCBHC) is designed to improve timely access to a full range of mental health and substance use treatment services offered to patients and clients regardless of insurance through continuous quality improvement and strategic community partnerships.

Position Highlights

  • Base hourly rate: $24.32 + (commensurate with experience)

What you will do

  • Engage consumers/clients in their care and offer appropriate and timely education to consumers, families and caregivers to facilitate understanding of service delivery and what to expect with the CCBH.
  • Facilitate patient-centered care that is compassionate, appropriate, effective and efficient
  • Improve the care coordination process through connections to the team, quality improvement and advancement of the profession
  • Coach and mentor individuals to meet their personal treatment goals.

Responsibilities

  • Assist clients in navigating the MBH and larger mental health and substance use treatment system
  • Engage individuals in their treatment
  • Collaborate with staff and family to achieve desired treatment outcomes.
  • Document the services provided
  • Contribute to the grant expectations that relate to the Care coordinator role
  • Support the collection of data that assists the program with measurement-based care

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor’s Degree in Social Work or related field preferred. 
    2.  License/Certifications: N/A 
    3.  Experience: Two years of experience working with behavioral health consumers and families preferred. Experience in a team based setting and collaborating with multiple providers preferred. 
    4.  Additional Skills/Requirements Required: N/A 
    5.  Additional Skills/Requirements Preferred: Knowledge about community resources.
