Care Transitions RN - Dementia specialty

This is a temporary, grant-funded position through August 31, 2025.

Are you passionate about supporting individuals with dementia and improving the care available to them? As we expand our dementia-capable services, join us as a dementia-focused care transitions RN.

Engage with patients in the crucial transition from hospital stay back to home. Collaborating with our inpatient care managers and our community partner, Southern Maine Agency on Aging, you'll help to improve the health and wellbeing of patients with dementia and ensure continuity of care.

Seeking an RN who thrives in an independent work environment, is creative and flexible, and has experience working with people living with dementia. This role is dynamic, literally and figuratively meeting the patient where they are – bedside at MMC & SMHC, via phone, and sometimes in person at the patient’s home (though this is not home care nursing).

This is a part-time, 24-hour/week, grant-funded position. Schedule is 3 weekdays/week, 8 am-4:30 pm. Once fully trained there is an option to work a hybrid remote schedule.


Summary:
The Registered Nurse – Care Transition role is responsible for providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual (adult internal medicine and family practice) patient needs following discharge from the hospital.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Graduate of an accredited School of Nursing required; Bachelor’s Degree preferred.

2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required.

3. Experience: Four years of experience in a hospital or medical office setting preferred.
